Intensive Case Management
Case management is a service that is available to families and individuals who want assistance in reaching their employment and educational goals in order to become self-sufficient. A full intake assessment and referral is completed on the household. This gives the case manager a better picture of the family’s situation. Strength’s based case management approach is used. Committing to case management means that you will work with a case manager each week towards your employment and/or educational goals. Appointments are scheduled in a location where the individual/family feels comfortable. Goals are identified through collaboration with the family. Referrals are made to other community resources and programs that the individual or family may need.
